Primary clear cell carcinoma of the liver, which has a clear cell cytoplasm, is an uncommon subtype of hepatocellular carcinoma (HCC). We report a case of a huge clear cell type HCC with necrotic change. A 71-year-old woman with a huge cystic tumor in the right lobe of the liver underwent careful observation for 3 years because the biopsy did not indicate malignancy. Thereafter, a few nodules appeared around the main cystic tumor and the biopsy was again positive for HCC. We performed right hepatectomy. The tumor was 10 cm in maximum diameter and simple nodular type. Although the main cystic lesion was necrotic, most of the wall of the cyst and surrounding nodules showed a clear cell variant of HCC. The background liver tissue showed no inflammation or fibrosis. This case is worth reporting because of its rare pathology with natural necrotic change of clear cell type HCC.
