Abstract
Neck dissection plays an important part in the treatment of papillary thyroid cancer. It is a relatively indolent disease. Therefore, neck dissection (ND) should be done with excellent cosmetic and functional results.
We chose the method of neck dissection depending on the extent of disease. Jugular ND or Modified ND is used to the lateral component of the neck. On jugular ND, upper, middle and lower jugular nodes and supraclavicular nodes are removed. On modified ND, dissection of nodes of posterior triangle is added. Of them, operative technique of jugular ND is mainly reported.
Dissection is done sharply using knife and sufficient counter-traction of the tissue. Dissected tissue is compared to a hexahedron and six aspects are dissected in order.
We performed eleatively the jugular neck dissections for N0 44 patients with papillary carcinoma of the thyroid. The pathological metastatic rate was 75% in the jugular nodes.
There is no recurrence case with the dissected or the undissected side of the neck.
Therefore, the jugular neck dissection is a reasonable and safety method and also has few complication for the patients with thyroid cancer.
