We reported a teenage girl who had experienced intermittent, sudden-onset, pulsating frontal headaches accompanied by nausea since elementary school. The headaches were not associated with focal seizures, and several episodes each year were severe enough to interfere with school attendance. EEG obtained during headache onset showed sharp waves broadly distributed over the right hemisphere. We made a diagnosis of ictal epileptic headache (IED) . Levetiracetam led to complete resolution of headaches and EEG abnormalities. The condition did not recur over four years.
