Abstract
When the tooth size ratio of the upper and lower anterior teeth do not match, it may be difficult to formulate an effective treatment plan. Particularly in cases whereby a patient has fused teeth, it is difficult to achieve aesthetic treatment goals. The patient was a 17-year-old male at the first visit, with a case of Angle class I crowding; the mandibular right central and lateral incisor teeth were fused. The fused tooth width measured 9.0 mm. The results of simulation using a setup model with the TOP analyzer system showed that opening a gap in the mesiodistal area of the fused tooth and restoring it as two teeth with laminate veneers would offer an effective aesthetic treatment. For orthodontic treatment, four upper and lower first premolar teeth and four third molar teeth were extracted, and upper and lower lingual braces were used. After leveling was completed, the maxilla was retracted using TADS. Post orthodontic simulations of aesthetics and functionality were performed with a temporary mock-up and a laminate veneer restoration was adopted with high patient satisfaction. As a result, we conclude that laminate veneers, which restore fused teeth to appear as two teeth, are considered to be a very effective treatment option.
