A case of sclerosing odontogenic carcinoma metastasized to the iliac bone

Abstract

 Sclerosing odontogenic carcinoma(SOC)is a rare tumor defined by the 2017 WHO classification of odontogenic tumors. No case of distant metastasis has been reported. Here, we report a case of SOC metastasized to the iliac bone after resection of the primary tumor. A 74-year-old woman visited our department with the chief complaint of pain in the right mandible. Radiographs showed an osteolytic lesion with an unclear boundary in the right mandible. A biopsy from the mandible yielded a pathological diagnosis of SOC. Therefore, neck dissection, a mandibular segmentectomy, and reconstruction with a fibular flap were performed. Since the resection margins were negative and there was no lymph node metastasis, adjuvant therapy was not performed. Three years and five months after the surgery, the patient complained of pain in the right iliac bone, and CT imaging showed an osteolytic lesion. Biopsy from the iliac bone confirmed the SOC metastasis. A cancer multi-gene panel test did not find any therapeutic agents suitable for treatment and the patient did not desire further treatment. She continues to be followed up while receiving denosumab. One year has passed without rapid growth of metastasis..
