A 20-year 3-month man who suffered severe asymmetry in the middle and lower third of face was presented. Simultaneous mobilization of maxilla and mandible (Le Fort-I Osteotomy+ Sagittal Splitting Osteotomy) was performed to flatten occlusal plane and after 6 months Genioplasty was done to correct remaining facial asymmetry.
Although minimum skeletal relapse occurred quite early after the first operation, desired facial symmetry was acquired.
