Sideroblastic Anemia with Skull Bone Lesion Resembling Thalassemia

Abstract
We report a 10-year-old girl with sideroblastic anemia who showed striking radiographic abnormality in the skull, “hair-on-end pattern, ” resembling thalassemia, and other hemolytic anemias. Head circumference was markedly increased and frontal bossing was recognized. Bone marrow examination revealed increased number of ringed sideroblasts. Further examination showed the decreased activity of 8-aminolevulinic acid synthetase. Hair-on-end pattern, representing chronic extreme increase in hematopoiesis, was previously reported in many cases with several congenital hemolytic anemias. So far as we reviewed, this will be the first report of a case of sideroblastic anemia with “hair-on-end pattern” in the skull X rays.
