Abstract
We report a case of huge duodenal gastrointestinal stromal tumor (GIST) with a liver metastasis. We needed to perform pancreatoduodenectomy with right hemicolectomy to remove the tumor in one block. We also performed radio frequency ablation (RFA) to the metastatic liver tumor. A 69-year-old woman with complaint of appetite loss was found to have an abdominal tumor. We made diagnosis of duodenal tumor that was suspected to be duodenal GIST with liver metastasis and performed an operation. The resected specimen was 13 × 11 × 8.5cm in sige and was elastic hard tumor with dark purple surface and was immunohistologically diagnosed as GIST. She was discharged from the hospital 40 days after the operation and shows no recurrence in 6 months of follow-up. As the success fully resected case duodenal GIST in one block, this is the largest one reported as for as we know. There are some reports recently concerning the effectiveness of RFA for the treatment of the levier metastasis from GIST. We believe RFA is also one of the effective methods in reducing the invasion of surgery. However, it may be necessary to examine the validity of indication of RFA for the liver metastasis from GIST in the future, because it differs histologically and from the view point of angiogesesis from usual hepatocellular carcinoma.
