Abstract
We report a case of solitary duodenal metastasis from renal cell carcinoma twelve years after nephrectomy. A 78-year-old man, who had undergone left nephrectomy at 66 years of age, presented with melena. On upper gastrointestinal endoscopy, a submucosal mass with ulceration was detected in the bulbus of the duodenum. Biopsy of the lesion demonstrated metastasis from renal cell carcinoma. CT scan showed an enhanced mass at the duodenum and the head of the pancreas. The patient underwent pancreatico-duodenectomy in October 2007. On histopathological examination, the tumor consisted of clear cell carcinoma, and the tumor mainly involved the muscle layer to the mucosa of the duodenum and had not invaded the pancreas. Based on these findings, we diagnosed duodenal metastasis, not pancreatic metastasis from renal cell carcinoma. Duodenal metastasis from renal cell carcinoma is very rare. We discussed this case along with a review of the literature.
