Abstract
A 50-year-old woman was admitted to our hospital because of a gastric submucosal tumor detected by medical examination. Preoperative abdominal CT scan showed an extra-gastric tumor 6cm in diameter and we diagnosed it as gastrointestinal stromal tumor (GIST) of the stomach. The tumor was connected to the greater curvature of the stomach with an extra-gastric pedicle and we performed laparoscopic partial gastrectomy. The tumor was resected with the gastric wall 2cm from the pedicle. Histropathologically, the tumor consisted of spindle cells. Immunohistochemically the tumor was positive for c-kit and was diagnosed as GIST. There were few mitotic figures less than 1/50 HPF. There have been no signs of recurrence as of 12 months after the operation. GISTs of the stomach with an extra-gastric pedicle are comparatively rare and only 31 cases have been reported in Japan. The prognosis of the GIST of the stomach with a pedicle is good for the size of the tumor compared with other gastric GISTs. The first choice of the treatment of the GIST is operation and it is preferable to perform partial gastrectomy including the normal gastric wall.
