Abstract
We report a rare case of ileal duplication presenting with peritonitis. A 35-year-old male developed diarrhea and lower abdominal pain. He had abdominal tenderness with guarding in the hypogastric area. Abdominal CT showed an inflammatory mass in the midline of the lower abdomen. Surgery was performed under the diagnosis of peritonitis and intra-abdominal abscess following acute appendicitis. During surgery, a cystic lesion was found in the mesentery of the ileum, 50 cm cephalad to Bauhin's valve. The ileum, the duplication, and the abscess were removed. On histology, the lesion was diagnosed as an ileal duplication, with a heterotopic pancreas located in the muscle layer of the duplication.
