A case of simultaneously both hepatocellular carcinoma and intraductal papillary mutinous carcinoma with postoperative pyelitis and ureter cancer

Abstract

A 68-year-old man who had been followed for increasing of aminotransferase level was admitted to the hospital because a liver tumor was found by abdominal US. He had undergone right nephrectomy for pyelitis and ureter cancer at the age of 62. Based on abdominal imaging findings, we diagnosed the patient as having both hepatocellular carcinoma (HCC) and intraductal papillary mucinous neoplasm (IPMN) grater than 3cm in diameter. The ICG R15 was 11%. Due to poor liver function reserve, we performed anterior resection of the liver, followed by pylorus preserving pancreaticoduodenectomy after an interval of 7 months. Histopathological examination showed that the liver tumor was moderately differentiatiated HCC with chronic hepatitis of simple nodular type with extranodular growth, and the pancreas lesion was IPMC with mucin hypersecretion of mixed type.
We report a rare case of simultaneously both HCC and IPMC with postoperative pyelitis and ureter cancer.
