A modified procedure for inguinal hernia tension-free mesh-plug repair using minimum local anesthetic solution

Abstract
Here we report our modified procedure for inguinal hernia repair by tension-free mesh plug technique using minimum local anesthetic solution in adults. The local anesthesia for adult patients with reducible inguinal hernia is safer especially for elderly patients suffering from cardiac or pulmonary diseases, and is preferred for those under anti-coagulation treatment. During the last 4 years, 20 tension-free groin hernia repairs were performed under local anesthesia. We have used a mixture of 1% lidocaine and either of 0.25% bupivacaine or 0.375% ropivacaine, or 0.375% ropivacaine alone as local anesthetic solution. Except cases using 0.375% ropivacaine alone, total volume of the anesthetic mixture came to approximately 8 to 18 ml (mean of 12.9ml). Our procedure is safe, simple and useful in terms of recognition of the local anatomy, and even inexperienceced surgeons can easily master the procedure.
