Abstract
Undifferentiated cancer of the stomach is defined as “cancer which does not include any portions showing differentiation toward adenocarcinoma and squamous cell carcinoma”. The case involved a 55-year-old man who was seen at the hospital because of upper abdominal pain and abdominal distention. Gastric endoscopy revealed a type 3 tumor at the pyloric antral zone of stomach. Biopsy of the lesion demonstrated poorly differentiated adenocarcinoma (por). Furthermore an abdominal CT scan showed the tumor invading the abdominal wall and multiple metastatic lesions of the liver. After admission chemotherapy with S-1/CDDP was immediately started, however, bleeding from the tumor became severe. So we had to perform distal gastrectomy, lateral segmentectomy of the liver and partial resection of the abdominal wall. On and after the 7th postoperative day, bloody ascites involving atypical cells appeared and increased in volume. The patient died of DIC and hepatorenal failure on the 39th postoperative day. On immunohistochemistry, the tumor cells were positive for cytokeratin and negative for AFP, CEA, EMA, MUC2, MUC5AC, MUC6, CD56, synaptophysin, and chromogranin A, Undifferentiated adenocarcinoma of the stomach was finally diagnosed.
