Abstract
A 56-year-old woman was admitted because of abdominal pain. Abdominal CT scan showed duodenal wall thickening, retroperitoneal emphysema, and a fluid collection. A retroperitoneal abscess secondary to perforation of the duodenal diverticulum was diagnosed, and emergency surgery was performed. A necrotized diverticulum was found in the second portion of the duodenum, and an enterolith was seen in the inner part of the diverticulum. After dissection of the diverticulum, the greater omentum was closed, and a decompression tube was passed.
Perforation of a duodenal diverticulum by an enterolith is rare and has characteristic features on CT scan.
