A case of giant malignant fibrous histiocytoma originating from the posterior peritoneum

Abstract
The patient was a 47-year-old man who consulted the Department of Surgery of our hospital for abdominal flatulence, swelling from both the lower extremities to the scrotum, and subsequent pain from the right flank through the back. Diagnostic imaging using multiple methods revealed a giant mass occupying the entire peritoneal cavity. Histological findings for a biopsy sample obtained via peritoneal puncture revealed storiform-myxoid-type malignant fibrous histiocytoma (MFH). He underwent surgery in July 2010. The tumor had numerous fluid-filled septal structures. The digestive tract and the right kidney were compressed toward the left part of the spine. The tumor was resected in several portions. A part of the tumor that was in contact with the lower plane of the right hepatic lobe and the posterior peritoneum could not be removed completely because of strong adhesion due to infiltration. After removal of the tumor tissue (weight, 6.2kg), the digestive tract returned to its normal position. A diagnosis of myxoid-to-pleomorphic MFH was made on the basis of postoperative histopathological examination. Cases of giant MFHs originating from the posterior peritoneum are rare in Japan. We report the case herein, with reference to the relevant literature.
