Abstract
Larrey hernia is rare type of retrosternal hernia. A 75-year-old woman developed symptoms of heart failure. Computer tomography (CT) scan revealed herniation of the greater omentum into the left pleural space through a Larrey foramen. Repair was performed using a laparoscopic approach. The size of hernia orifice was 50 × 35 mm. The greater omentum in the sac was easily reduced into the abdominal cavity. The defect was primary closed with direct sutures without excision of the hernia sac or using reinforcement materials. The symptoms disappeared completely and she was discharged on the eighth postoperative day without any complications. At a follow-up examination she had no sign of recurrence, although a CT scan showed minor fluid accumulation in the hernia sac.
This procedure is minimally invasive and permits early postoperative ambulation, so this method should be considered as the first choice for treatment of Larrey hernia.
