Abstract
An 80-year-old man was seen at the hospital because of anal bleeding. Colonoscopy disclosed a type 1 tumor 35 mm in diameter at the upper rectum. Biopsy resulted in adenocarcinoma associated with squamous differentiation. Laparoscopic-assisted low anterior resection of the rectum (D3 dissection) was performed. The histopathological diagnosis was adenosquamous carcinoma, pSS, int, INFb, ly3, v2, pN2, and pstage IIIb. Adjuvant chemotherapy with UFT/LV regimen was started in the 4th week after the operation. The patient developed multiple liver metastases 8 months after the operation, and then he was treated with bevacizumab+CapeOX, bevacizumab+FOLFIRI, and panitumumab+FOLFOX regimen. However, he died of multiple liver metastases 2 years after the operation.
Adenosquamous carcinoma is extremely rare and represents about 0.1% of all cancers of the large intestine. It is believed that the disease carries a poor prognosis. Since this patient was also to survive for two years by repeated adjuvant chemotherapies, we present the case, together with a review of the literature.
