Abstract
We report a rare case of Ewing's sarcoma / primitive neuroectodermal tumor (ES/PNET) arising from the lung and attached to the pleura by pedicles.
A 29-year-old man was admitted to our hospital because of an abnormal shadow on a chest roentgenogram. Chest CT scan showed a heterogeneous solid mass in the S8 segment of the right lower lobe of lung which was adjacent to the chest wall. Solitary fibrous tumor was likely. Tumor resection concomitant with partial pulmonary resection was performed. The mass was a pedunculated reddish brown tumor projecting from periphery portion of the right lower lobe of the lung. It was easily resected. The surgical specimens showed a small round cell tumor with positive staining for MIC2 gene product (CD99). EWS-FLI1 fusion transcripts were detected by reverse transcriptase polymerase chain reaction (RT-PCR), confirming the diagnosis of ES/PNET of the lung. In spite of post-operative adjuvant chemotherapy, solitary pulmonary metastasis occurred 18 months after the surgery. After metastasectomy and chemotherapy, the patient has been free from relapse for 3.5 years.
