A case of axial torsion of big Meckel's diverticulum

Abstract
We report a case of axial torsion of big Meckel's diverticulum. A 49-year-old man was admitted with complaints of abdominal pain around his navel. An abdominal CT scan showed a dilated enteric canal that looked like a cisterna around the right lower abdomen and a whirl sign of the mesenterium. Emergency laparotomy was performed under the suspicion of torsion of the small intestine. A Meckel's diverticulum, 11 cm × 10 cm in size, was found on the ileum approximately 75 cm proximal to the Bauhin's valve. It was twisted approximately 360° counterclockwise at its neck. Diverticulectomy was then performed without partial resection of the ileum. Axial torsion at the neck is a rare complication of Meckel's diverticulum ; to our knowledge, a total of 20 such cases, including ours, have been reported in Japan previously.
