Four cases of adult umbilical hernia due to massive ascites associated with hepatic cirrhosis repaired under local anesthesia

Abstract
Umbilical hernia in an adult leads to incarceration if elective surgery is not performed. It is important that elective surgery is done safely to avoid incarceration. This paper describes four adult patients with liver cirrhosis and a large quantity of ascites. Case 1 : 49-year-old woman, case 2 : 42-year-old man, case 3 : 73-year-old man, case 4 : 47-year-old woman. Only case 3 had type C hepatitis with cirrhosis, the others were alcoholic cirrhosis, and all patients underwent elective surgery. A circle-shaped skin incision was made on the abdomen surrounding the navel as the center, and the hernia sac along with the navel was dissected out under local anesthesia. Ventralex Hernia Patch® (VHP) was inserted into the peritoneal cavity through the defect and was stitched to the edges of the hernia defect. There were no serious complications, although one patient had leakage of ascites. There was no recurrence within one year after the operation. Therefore, we were able to perform elective surgery safely using VHP for patients who had liver cirrhosis with a large quantity of ascites.
