Abstract
The patient was a 16-year-old man who had been treated intestinal malrotation with midgut volvulus by the Ladd's procedure at 3 days of age. He was admitted to our hospital because of the abrupt onset of abdominal pain and vomiting 16 years after the procedure. Enhanced computed tomographic scanning detected a SMV rotation sign and a whirl-like pattern of the twisted mesentery. He was diagnosed with recurrence of midgut volvulus due to intestinal malrotation and underwent an emergency laparotomy. The mesentery was twisted by 540 degrees in a counterclockwise fashion. The ascending and descending colons were not fixed to the retroperitoneum. The intestine was viable and no bowel resection was required. He recovered satisfactory.
Recurrence of midgut volvulus is extremely rare, but complications can emerge after the Ladd's procedure. We recommend the fixation of the intestines in addition to the Ladd's procedure in order to prevent the recurrence of midgut volvulus.
