Abstract
A 49-year-old man without a previous history of laparotomy, developed acute abdominal pain. Abdominal computed tomography revealed a small bowel obstruction. However, the cause of obstruction was unknown. Strangulation was suspected, and emergency laparoscopic surgery was performed. An orifice made by an adhesion between the appendix epiploica of the sigmoid colon and the left lateral umbilical fold was found, and the small bowel was incarcerated in the orifice. We laparoscopically resected the adhesion and released the small bowel. The blood flow to the small bowel improved, and a bowel resection was not performed. The patient was discharged four days after surgery without complications. Reports of small bowel obstruction caused by appendix epiploica of the sigmoid colon are rare, and only 11 cases have been reported in Japan. No cases have been treated by laparoscopy alone, except the case reported here.
