A 65-year-old man admitted to our department for the treatment of a lower abdominal mass. Abdominal CT showed a solitary cystic mass measuring 13 cm in diameter in the presacral space. An enhancing nodule was detected in the wall of the cyst. A PET/CT also revealed a nodule measuring 2 cm in diameter in the same area of the cyst, that showed a maximal SUV of 1.5. Therefore, the cystic mass was resected via the sacral approach in the jack-knife position. Histopathology revealed that the cyst was lined by squamous epithelium, and the nodule proved to be a squamous cell carcinoma. We encountered a rare case of squamous cell carcinoma arising from a presacral epidermoid cyst. Once diagnosed, surgical resection is mandatory because of the malignant potential of the tumor. We report this rare case with a review of the literature.
