Abstract
An 87-year-old man complaining of abdominal distention was diagnosed with obstructive sigmoid colon cancer and placed a self-expandable metallic stent (SEMS) urgently at another hospital. Four days later, he called for first aid because of left lower abdominal pain. Gastrointestinal perforation was diagnosed and he was transferred to our hospital for the purpose of operation. Sigmoidectomy and colostomy were performed, because we found colonic perforation in contact with the oral end of SEMS during an emergency operation. The postoperative course was uneventful, but the patient died of cardiac insufficiency resulting from worsened myocardial infarction, that was his underlying disease, on the 65thday after the operation. SEMS treatment which became the insurance adaptation for obstructive colorectal cancer in Japan in 2012 has been recognized as the standard treatment rapidly. However, it is important to use SEMS or trans-anal ileus tube for different purposes, because SEMS placement is not always safe as the procedure.
