A Case of Primary Epithelioid Hemangioendothelioma of the Liver with Pulmonary Metastases Successfully Resected via Repeated Hepatectomy

Abstract
Epithelioid hemangioendothelioma (EHE) is a relatively rare low-grade nonepithelial tumor arising from the vascular endothelium. Herein, we report a case of primary EHE of the liver, in which the primary tumor and postoperative intrahepatic recurrence were successfully resected. The patient was a 61-year-old man who was examined for abdominal pain. Multiple liver tumors were identified and diagnosed as EHEs on tumor biopsy. Multiple metastatic lesions were observed in both the lungs, but the lesions were stable. After the preoperative diagnosis of primary EHE of the liver and multiple lung metastases, partial hepatectomy was performed. Pathological examination of the resected specimens from the liver showed proliferation of atypical epithelioid cells with rich fibrous stroma. The specimens were immunohistochemically positive for CD31 and CD34. Based on these findings, the tumors were finally diagnosed as EHEs of the liver. During postoperative follow-up, a solitary liver metastasis was observed in the remaining right lobe. The multiple lung metastases were still no change in size, while the recurrent liver tumor had enlarged. Therefore, the remaining right lobe of the liver was also resected. The tumor was pathologically diagnosed as an intrahepatic recurrence of EHE. Six months after the second surgery, the patient remains recurrence-free in the liver.
