Abstract
A 66-year-old man with gastric cancer underwent laparoscopic total gastrectomy with Roux-en-Y reconstruction. On postoperative day 3, the patient was referred for high fever and elevated inflammation markers. Upper gastrointestinal series revealed major anastomotic leakage with a large abscess cavity in the right upper abdomen. On postoperative day 8, we started per-nasal transesophageal abdominal drainage by endoscopy with fluoroscopic examination. This therapy was performed for a total of 12 days. After termination of therapy, both control endoscopy and computed tomography confirmed closure of the leakage. He was discharged on day 38. In conclusion, this is a noninvasive and effective procedure that can be applied to otherwise therapy-refractory abdominal anastomotic leakage after laparoscopic total gastrectomy.
