Abstract
A 51-year-old man was referred to our hospital for treatment for a retroperitoneal tumor. Enhanced abdominal computed tomography (CT) revealed a well-defined tumor, 66×48 mm in diameter, on the right side of the pelvic cavity with compression of the right external iliac vein. 3D-CT angiography visualized numerous feeding vessels behind the tumor, located between the internal iliac artery and vein. The tumor was resected with laparoscopic surgery for diagnosis and treatment. Extensive adhesion was found between the tumor and the right external iliac vein. The tumor was resected, with the feeding vessels divided safely using a surgical sealing device. On histology, Castleman disease of the hyaline-vascular type was detected. Although retroperitoneal pelvic Castleman disease is rare, it can be safely resected laparoscopically with precise information of the tumor obtained by preoperative 3D-CTA.
