Abstract
We present a rare case of perforation of a colostomy stoma caused by extraction of feces with chopsticks.
A 69-year-old man who had undergone Miles' operation for rectal cancer in 1998 had been controlling his artificial anus by himself by colostomy irrigation every morning for 15 years. He was admitted to our hospital with abdominal pain after irrigation in January 2013. Abdominal computed tomography showed abscess with free air around the colostomy stoma. Emergency surgery was performed under a diagnosis of peritonitis due to perforation of the colostomy stoma caused by irrigation. The perforated site was located about 10 cm oral to the opening of the artificial anus. We resected the artificial anus including the perforated site and reconstructed the stoma. He was discharged on postoperative day 27. We confirmed that perforation had been caused by extraction of feces with chopsticks after eliciting a detailed medical history.
