Abstract
A 75-year-old woman presented with erosion of the perianal skin. A skin biopsy showed the erosion to be Bowen's disease, and she was referred to our hospital to be treated. The lesion spread in the range of a half circle of the anal right side and reached the anal verge. Wide local excision with a 6-mm margin on the anal mucosal side and a 10-mm margin on the skin side was performed under general anesthesia. We checked the stumps of the mucous and skin sides in the rapid pathological diagnosis during surgery, and it was found to be negative. The defect after resection was reconstructed via dermal flap formation.
The standard treatment of perianal Bowen's disease is wide local excision of the lesion. Intensive follow-up is needed after surgery because of the high recurrence rate (16%-25%). We believe that an intraoperative pathological rapid diagnosis can ensure a safe resection line.
