Three Cases of Abdominal Trauma where Surgery under Combined Intra-aortic Balloon Occlusion was Effective

Abstract
We report three cases of intra-abdominal hemorrhage caused by abdominal trauma that were treated effectively using intra-aortic balloon occlusion (IABO) for descending aortic cross-clamping prior to laparotomy. Case 1 involved an 18-year-old man with damage to the spleen and kidney sustained in a traffic accident, for which preoperative IABO was performed to stabilize the circulatory dynamics. Hemostasis, splenectomy, and nephrectomy were performed under laparotomy. Case 2 involved a 46-year-old man with abdominal puncture wounds caused by self-mutilation. IABO was performed prophylactically before laparotomy. During laparotomy, a puncture wound was observed in the left hepatic lobe, and suture hemostasis was performed with a favorable field of view while bleeding was being controlled by IABO. Case 3 involved a 40-year-old man with mesenteric damage caused by a traffic accident, for which preoperative IABO was performed to stabilize the circulatory dynamics, and hemostasis under laparotomy was performed. Renal function deteriorated transiently after surgery, but later recovered. IABO can help stabilize preoperative circulatory dynamics, and is effective in contributing to intraoperative bleeding control, ensuring a favorable field of view, and hemostasis. However, careful attention must be paid to complications caused by ischemia.
