Abstract
[Aim]We report on the technique and operative results of an advanced laparoscopic percutaneous extraperitoneal closure method (LPEC with iliopubic tract repair to reinforce the wall of the inguinal canal) in an alternative surgical procedure in which mesh sawing is not performed. [Patients & Method]We performed the advanced LPEC method in the treatment of six young adult patients with a relatively small indirect inguinal hernia (JHS : I-1∼2). We obtained informed consent before the operation in all patients. First, we performed a laparoscopic puncture between the aponeurotic arch of the transversus abdominis and the iliopubic tract using the LPEC needle from the abdominal side for reinforcement. We then performed the standard LPEC procedure. Finally, the wall of the inguinal canal was sutured. [Result]Six patients underwent surgery with the advanced LPEC method. Although the follow-up period has been short, thus far (17.7 months after surgery), none of the patients have developed a recurrent hernia. [Conclusion] The advanced LPEC method is considered to be a highly effective procedure if limited to the treatment of young adult patients with an indirect inguinal hernia.
