Abstract
We report a case of primary ileal cancer requiring emergency surgery, with the complication of jejunal intussusception due to an ileus tube. In March 2015, a 74-year-old man consulted a prior physician for abdominal discomfort persisting for over a month. He was later referred to our hospital for further examination, and was admitted to our hospital with a diagnosis of ileus from CT. The cause of obstruction was suspected to be in part of the ileum imaged on CT at that time. An ileus tube was used to reduce pressure in the small intestine. After further examination of the cause of obstruction, we decided on a strategy of elective surgery. However, the ileus tube was not readily advanced, failing to sufficiently mitigate the ileus, and further examination of the cause of obstruction was impossible. The ileus tube was therefore advanced under radiographic guidance on hospital day 14. This was followed by sudden abdominal pain, fever, and a drop in blood pressure. Jejunal intussusception was suspected from CT, and emergency surgery was performed in the form of intussusception reduction and partial resection of the ileum. The postoperative course was favorable. Primary ileal cancer was diagnosed from postoperative pathological testing. This report also includes a short discussion of the literature.
