Abstract
Perforation of the stoma limb is an extremely rare complication that has been reported in only few cases so far. It can be caused by maneuver for self-care of the stoma, fecal impaction, and penetration of cancer. We have experienced a case of perforation of sigmoid colostomy stoma limb caused by fecal impaction. The case involved a 61-year-old man who presented to our hospital because of the abrupt onset of abdominal pain one year after abdominoperineal resection. There was strong tenderness around the stoma. An abdominal CT scan revealed an image which appeared to be fecal mass in the abdominal wall around the stoma. Emergency operation showed a cyst including a hard fecal mass in the abdominal wall, adjacent to which a perforation was present in the sigmoid colostomy stoma limb. By performing operations via the wound through which the stoma had been drawn, we resected the affected colon and created another wound for reconstruction of stoma. He developed wound infection, but otherwise the postoperative course was uneventful. He was discharged from the hospital on the 14th postoperative day. Histopathological studies revealed absence of diverticulum and tumor, and stercoral colon perforation was diagnosed.
