Abstract
Spermatic cord metastasis from colorectal cancer is very rare and carries poor prognosis. We present a case of left spermatic cord metastasis 57 months after surgery for rectal cancer. A 59-year-old man had low anterior resection for stage IIIa rectal cancer followed by adjuvant chemotherapy in September 2009 when he was 54 years old. He complained of a painful left-sided inguinal mass in June 2014 (4 years and 9 months after the operation). A contrast-enhanced CT scan showed an enhanced tumor in the left spermatic cord. He was diagnosed with spermatic cord metastasis and received left orchiectomy in July 2014. The patient's postoperative recovery was good. Microscopic findings showed moderately differentiated adenocarcinoma compatible with metastasis from the rectal cancer. He was able to receive adjuvant chemotherapy early, but died 16 months after the orchiectomy. So far only nine cases, including ours, have been reported in Japan.
