Abstract
A 50-year-old woman had an endoscopy and a computed tomography scan, and these examinations revealed a gastric submucosal tumor and a metastatic liver tumor, respectively. The patient had a history of gastroduodenal ulcer, which was treated conservatively. Prior to surgery, the tumor was diagnosed as a primary gastric gastrinoma with liver metastasis. Therefore, distal gastrectomy with D2 lymphadenectomy and lateral segment hepatectomy were performed. Although postoperative serum gastrin levels decreased temporarily, 4 months after surgery these levels had elevated. Since the patient showed recurrence of metastatic disease, she received molecular-targeted therapy.
The majority of gastrinomas are located in the gastrinoma triangle. Ectopic gastrinoma is a rare disease, and there have been few cases of primary gastric gastrinoma. Patients with liver metastasis from gastrinoma have a poor prognosis. In these patients, in addition to surgery, molecular-targeted therapy and selective hepatic artery embolization are reported to be promising treatment strategies. Herein, we report a rare case of primary gastric gastrinoma with liver metastasis, taking into consideration, the previous literature findings.
