Abstract
An 88-year-old woman admitted to our hospital because of acute abdominal pain was diagnosed as having intestinal obstruction near the terminal ileum and a cystic mass in the pelvis based on abdominal CT findings. Although causal relationship between strangulation ileus and the tubular structure was obscure, emergency surgery was performed with the diagnosis of strangulation ileus. The tubular structure on imaging was found to be a cystic tumor of the appendix and the ileum was squeezed and strangulated by the appendix itself, led by the cyst. The cyst squeezed around the small intestine, causing ischemia. Ileocecal excision was performed. The cystic mass was diagnosed as low-grade appendiceal mucinous neoplasm, pathologically. Strangulation ileus caused by the appendix led by a low-grade appendiceal mucinous neoplasm is extremely rare. We report this case here.
