Abstract
We report a rare case of incarceration of the transverse colon in the esophageal hiatus presenting with duodenal obstruction. A 77-year-old woman was admitted to a local hospital with prolonged anorexia and vomiting, and was then referred to our hospital. Abdominal computed tomography showed duodenal obstruction caused by traction on the duodenal bulb by the mesentery of the transverse colon incarcerated into the mediastinum. She was diagnosed with duodenal obstruction caused by incarceration of the transverse colon alone in the esophageal hiatus. Laparoscopic repair of the hiatal hernia was performed. The postoperative course was uneventful and no recurrence was observed in the 2 years after surgery. Esophageal hiatal hernias in which only the transverse colon was incarcerated have been reported ; however, there have been no reports of associated duodenal obstruction.
