Abstract
There are no specific therapies for chronic intestinal pseudo-obstruction (CIPO) and total parenteral nutrition (TPN) is mandatory. We describe a patient with CIPO who had been put on TPN for 20 years in whom we were able to place a CV catheter through the testicular vein. The case involved a 59-year-old man who underwent enterostomy for CIPO at the age of 40, and thereafter he was repeatedly admitted and discharged for catheter-related bloodstream infection (CRBSI). This time he was emergently admitted to our hospital because of CRBSI. After controlling his infection, we sought possible CV routes but most main vessels had been obstructed, so that we performed the venous cutdown technique from the testicular vein as an alternative route. The left inguinal region was opened under local anesthesia and a 16G CV catheter was inserted from the left testicular vein up to the inferior vena cava through the renal vein. The postoperative course is satisfactory as of 6 months after the TPN was resumed.
