A 36-year-old man diagnosed with Klinefelter's syndrome visited our hospital. A mass was found on his chest radiography. Computed tomography revealed a diaphragmatic hernia with fat tissues. Laparoscopic repair was performed for the hernia. The herniation occurred via the foramen of Larrey, and the size of the hernia orifice was 6 × 4 cm2. The defect was closed with a Composix Mesh® fixed with a hernia stapler. He was discharged from the hospital 3 days after surgery, and no recurrence was observed 3 weeks after the operation. Retrosternal hernias have been reported to account for only 1% to 3% of all diaphragmatic hernias. In addition, Larrey hernia accounts for 5% of all retrosternal hernias. We report a rare case of Larrey hernia coexisting with Klinefelter's syndrome.
