The patient was an 80-year-old woman. She underwent extended right hepatectomy and resection of the extrahepatic bile ducts 7 yeas ago. Histopathological diagnosis comfirmed well to moderately differentiated adenocarcinoma of the gallbladder. S-1 was administered for one year as adjuvant therapy. Contrast-enhanced computed tomography performed 5 years postoperatively revealed a tumor with an unclear border at the head of the pancreas. Endoscopic retrograde cholangiopancreatography revealed an occupied lesion in the bile duct and the pancreatobiliary maljunction. Recurrence of intrapancreatic biliary duct was suspected, and we performed subtotal stomach-preserving pancreaticoduodenectomy. Histopathological examination of the specimen confirmed adenocarcinoma similar to the findings observed in the previous specimen. Two years later, she developed a 2.5-cm sized lesion in the right kidney. Metastatic recurrence was suspected, and we performed translumbar right nephrectomy. Histopathologically, the tumor resembled the previous specimen, and we diagnosed gallbladder cancer metastasis. We report a rare case of renal metastasis in a patient with gallbladder cancer.
