A 56-year-old man presented with a right lateral abdominal bulge and was diagnosed with a right Spigelian hernia. The patient underwent laparoscopic right Spigelian hernia repair by the transabdominal preperitoneal approach. During laparoscopy, bilateral indirect inguinal hernias were identified in addition to the right Spigelian hernia. The right Spigelian hernia and inguinal hernia were repaired using two pieces of mesh. One was adjusted to the Spigelian hernia orifice and the other was original. The left indirect inguinal hernias were repaired routinely. Using laparoscopy, we can diagnose other hernias that were not found preoperatively and accurately measure the length of the hernia orifice.
