A 74-year-old woman presented to our hospital complaining of appetite and weight loss. CT showed a slightly enhanced tumor of the head of the pancreas, 19 mm in diameter, with portal vein invasion. Additionally, three-dimensional CT showed an aberrant right hepatic artery arising from the gastroduodenal artery (Type D aberrant). The patient had undergone pancreatoduodenectomy and portal vein resection. Pathologically, the tumor was diagnosed as pancreatic ductal adenocarcinoma. The incidence of type D was reported to be only 0.28%. However, this case serves as an important reminder to consider preoperative three-dimensional CT and intraoperative clamp testing of the gastroduodenal artery before resection to prevent aberrant right hepatic artery injury.
