A 15-year-old adolescent male sustained abrasions over his extremities after he fell off his bicycle. His vital signs and physical examination were unremarkable. However, he experienced sudden upper abdominal pain, 4 hours later. Contrast-enhanced computed tomography revealed a large volume of ascites and leakage of the contrast medium into a cyst (8 cm) at the upper pole of the spleen. He received immediate transfusion ; however, hemodynamic instability persisted, necessitating emergency dome resection of the splenic cyst. His postoperative course was uneventful, and he was discharged 18 days postoperatively. Splenectomy is associated with an increased risk of severe postoperative infections. Therefore, there is a clinical profit because we were able to preserve the bulk of the spleen.
