We report a case of invasive lobular carcinoma of the nipple in a 58-year-old woman who observed rashes on her right nipple, 3 months prior to presentation. She had rice grain-sized papules on the outside of the right nipple and at the 10 o'clock position on the right areola. Skin biopsy findings revealed adenocarcinoma. An area of induration (1.5 cm) was palpated around the right nipple ; however, mammography and ultrasonography revealed no breast lesions. Contrast-enhanced magnetic resonance imaging revealed a fast-plateau pattern contrast effect only in the right nipple. Imaging studies revealed no distant metastasis. She was diagnosed with cT1cN0M0 cStage IA breast cancer localized only to the nipple and underwent partial mastectomy, including nipple and areola excision. Histopathological and immunohistochemical findings showed invasive lobular carcinoma, 2 cm, ly0, v0, ER score 3b, PgR score 3b, HER2 score 1, Ki-67 score 8%. The tumor was localized to the nipple and radiotherapy was initiated, followed by oral anastrozole administration. No recurrence is observed to date, 10 months postoperatively.
