We report the case of a 69-year-old man who presented with bilateral, synchronous, encapsulated papillary breast carcinoma. Approximately 5 years prior to presentation, he developed a right breast mass, which showed gradual enlargement. Evaluation showed an intracystic tumor in the right breast and a solid, well-circumscribed tumor in the left breast. Core needle biopsy findings confirmed that both tumors were papillary carcinomas. The patient was diagnosed with bilateral synchronous breast cancer and underwent bilateral mastectomy and sentinel node biopsy for both breast tumors. Histopathologically, both tumors showed features of intracystic carcinoma ; however, myoepithelial cells in the papillary lesion and cystic wall were poorly immunostained, which led to diagnosis of encapsulated papillary carcinoma.
