A Case of Juvenile Idiopathic Chronic Pancreatitis with Marked Pancreatic Duct Dilatation with the Maximum Diameter of 12 mm and Pancreatic Stone

Abstract

A 34-year-old man who had never taken alcohol presented with epigastric to left back pain. No increases in pancreatic enzymes were observed, and the pain resolved spontaneously within 1 week. Ultrasonography and MRCP revealed marked dilation of the pancreatic duct and a lot of pancreatic stones. He was referred to us for surgical treatment. A small incision was made in the body of the pancreas, from where pancreatic stones were removed as much as possible, and pancreaticojejunal anastomosis and R-Y reconstruction were performed. In addition, cholecystectomy was performed for suspected gallbladder polyps. Histopathological examination revealed chronic pancreatitis.

Juvenile idiopathic chronic pancreatitis is relatively rare. Although it is not well known in Japan, there is a concept of “tropical calcifying pancreatitis”, and we report a case that may be applicable in the future.
