A young woman in 29 with a complaint of right lower abdominal pain visited a hospital. Acute appendicitis with an abscess was diagnosed, and interval appendectomy was planned. Antibiotic treatment was effective, but her symptoms recurred twice thereafter. The patient was thus referred to our hospital and underwent laparoscopic appendectomy. Pathological examination showed endometrial glands and CD10-positive stroma in the muscular to subserosal layers of the appendix, leading to the diagnosis of appendiceal endometriosis. Appendiceal endometriosis is a rare clinical entity whose clinical signs and symptoms are non-specific and not necessarily related to the menstrual cycle. In addition, there are no specific imaging findings of appendiceal endometriosis. Therefore, a preoperative diagnosis of appendiceal endometriosis is quite difficult to achieve. When a fertile female patient presents with repeated bouts of appendicitis, appendiceal endometriosis should be considered in the differential diagnosis.
