We analyzed the acceptance for fibrin glue free pleural covering after resection of bullae at thoracoscopic surgery of juvenile spontaneous pneumothorax by showing our selection criteria and the therapeutic results. A total of 43 initial spontaneous pneumothorax surgery patients of 29 year-old or younger (37 sides in men and 6 in women) were enrolled in this study. According to intraoperative findings, we assigned narrow (roughly less than 2 cm square) bullous cases to no fibrin use group. No fibrin glue use group was comprised of 23 cases. Recurrent rate of pneumothorax after surgery in no use group was 4.3% (one side). It was reviewed that only a recurrent case actually had a somewhat broader bullous lesion than our selection criteria. The procedure of fibrin glue free pleural covering seems to be acceptable for thoracoscopic surgery of juvenile spontaneous pneumothorax, when it is important to ascertain wether the bulla is localized.
