We report the case of an 89-year-old man with strangulating intestinal obstruction caused by a peritoneal defect following transabdominal preperitoneal (TAPP) repair. On postoperative day 12, the patient developed abdominal pain and vomiting. Computed tomography indicated herniation of the small bowel into the preperitoneal space with signs of strangulation. Emergency surgery revealed dehiscence of the peritoneal closure, leading to herniation and necrosis of the small bowel. A bowel segment was resected, and the peritoneum was securely re-closed. This case underscores the importance of peritoneal closure during TAPP procedures to prevent postoperative complications.
